Support Coordinator

We’re looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join the Associations Stroke Service in Surrey.

Position: Stroke Support Coordinator

Location: Home based Surrey, with regular travel across the 4 service areas within Surrey

Hours: 25 hours per week

Salary: Circa £16,500 per annum

Contract: This is a fixed term contract until 1 July 2021. Services are contracted; there is currently funding for this contract until 1 July 2021 with intent for contract renewal.

The Role

Reporting to the Stroke Support Manager, the Stroke Support Coordinator will:

 

  • Support stroke survivors and their carers from hospital into the community.
  • Provide personalised information, advice and support.
  • Provide regular reviews to support people in establishing and achieving their own personal goals.
  • Recruit, train and manage a team of volunteers that will support the service.

 

About You

Applicants are likely to have:

 

  • A background in a health or caring profession, ideally with experience of supporting people with disabilities and stroke.
  • Excellent IT skills and a strong understanding of how to use data to improve quality.

 

A flexible approach to your role is essential, as is willingness to travel. The use of a car with business use cover and the ability to drive is essential to be able to fulfil the requirements of the role.

Appreciating much of this role will be spent travelling in Surrey; the charity offers a Make Your Week Work for You and Those We Serve benefit, which in agreement with your line manager offers the opportunity to work flexibly. If you live within the service area, mileage can be claimed from your location base, if you live outside of your service area, mileage will be claimed from the service boundary. There are currently four service areas in Surrey which will be discussed further at interview and are regularly reviewed.
